Anyone else having this sort of image generation?

Looking at the image as a whole looks kinda grainy, something I've not had in ages. This sort of thing is happening in every generation since 2.0 came out. Is there a way to fix it? Look at the bombed building, there's clear places where the generation just isn't working (windows fading over the top of bricks etc White dots all over too. What's going on?
