While Classifier-Free Guidance (CFG) can improve instruction adherence, standard static implementations often cause semantic dilution and linguistic degradation. We propose SPREG (Structured Plan-guided Real-time Entropy Gating), a lightweight inference-time framework for surgical error rectification.
