Governance is the execution layer, not the policy memo

The pattern I keep seeing is teams treat governance like documentation while the agent is still free to do dangerous things at runtime. If the model can call tools, move money, message users, or touch production, governance has to live in the execution path. For me that means deterministic checks before tool use, explicit escalation boundaries, and telemetry good enough to reconstruct why an action happened.
